What goes in Box 15A
Total insurable hours
how do you calculate total insurable hours
- determine the pay period type
- determine the amount of pay periods between block 10 and block 11
- determine the number of hours up to the maximum p/.p. allowed (number pay periods for pay period type)
- determine the number of hours
- include vacation pay, ot, and stat unless after date in block 11
What is special about stat pay
do not include in insurable hours (15A) if it happens and is paid after the date in block 11 and the employee is NOT returning
what is in block 15B
total insurable earnings (should complete block 15c and 17 first)
What is in block 15C
insurable earnings
how do you calculate insurable earnings
final pay period goes in first field marked (1)
second last in field marked (2)
and so on
any nil amounts, enter 0.00 in the block
what goes in 17A
vacation pay
if vacation is included wit each pay where does it go
does not go in 17A
if paying vacation owed because no longer working there
include in box 17a
- no comment in block 18
if paid vacation after date in block 11 (ie paid on specific date(s) each year
enter in block 17A
- put the date(s) in block 18
what goes in 17b
stat holiday pay
- if it falls after the date in block 11 and you are paying the employee for it
What goes in block 17C
other monies such as bonus, pay in lieu of notice, severance pay, profit sharing gratutites etc
How do you calculate insurable earnings
add up all the amount sin 15C and enter the total in 15B
what goes in block 19
paid sick/ maternity etc
