Minor criteria for heart failure
bilateral ankle edema nocturnal cough dyspnea on ordinary exertion hepatomegaly pleural effusion decrease in vital capacity by 1/3 from maz recorded tachycardia >120 BPM
Major criteria for heart failure
Paroxysmal nocturnal dyspnea (nighttime dyspnea) JVD Cardiomegaly on CXR pulmonary edema S3 gallop increased central venous pressure hepatojugular reflex weight loss >4.5kg in 5 days
Class I heart failure
No limitation of physical activity. Ordinary physical activity doesnt cause undue fatigue, rapid/irregular heartbeat, palps, or SOB
Class II heart failure
slight limitation of physical activity. comfortable at rest but ordinary physical activity results in fatigue, rapid/irregular heartbeat, palps, SOB
Class III of heart failure
marked limitation of physical activity. comfortable at rest but less than ordinary activity causes fatigue. rapid/irregular heartbeat, palps, SOB
Class IV heart failure
unable to carry out any physical activity w/o discomfort. fatigue, SOB, rapid/irregular heartbeat, palps, present at rest
Stage A heart failure
pt at high risk for HF but without structural heart disease or symptoms of HF- ie HTN, DM, obesity, CAD, PVD, CVA, fh
Stage B heart failure
pts with structural heart disease but w/o signs and symptoms of HF
ie - prior MI, left vent Hypertrophy or valvular disease
Stage C heart failure
pts with structural heart disease with prior or current symptoms of HF
ie known structural heart disease and dyspnea, fatigue, reduced exercise tolerance
Stage D heart failure
pts with refractory heart failure requiring specialize interventions
ie pt with marked symptoms at rest despite max med therapy with recurrent hospitalizations
Cor Pulmonale
reight sided heart disease, secondarily caused by abnormalities of lungs
no evidence of other heart conditions
RF; smoking, predisposition for venous thrombosis, high altitude
Disease of lungs/airway: COPD, asthma, lung disease, TB, emboli
Sx: cyanosis, elevated JVD, hepatojugular reflex, S3 gallop, tricuspid regurg, ascites, SOB, fatigue, deceased exercise tolerance, wt. gain, increased peripheral edema
