State the attachments of the muscle

A

Proximal insertion, medial or Humeral head from the medial epicondyle of the humerus via the CET and anterior aspect of the Ulna collateral ligament and medial coronoid process
Lateral head for upper two thirds of anterior radius from radial tuberosity to insertion of Pronator Teres
Distal insertion, by four separate tendons attaches to the the palmar aspect of the middle phalanx of each digit via two lateral slips
