Step 1
Wash hands
Step 2
Introduce yourself and greets patient. Explains procedure

Identifies patient: asks name, spelling, DOB

Verifies diet restrictions and medication (blood thinners or fasting?)
Step 5
Selects collection tubes to correspond with test requests on requisition
Step 6
Places cotton ball, tubes in the order of draw and any other supplies needed within arm’s reach on non-dominant side
Step 7
Ensures sharps container is on dominant side to prevent crossing over
Step 8
Hand sanitize; puts on gloves
Step 9
Applies tourniquet and asks patient to clench fist; selects venipuncture site - palpates vein with non dominant hand
Step 10
Releases tourniquet
Step 11
Cleans the site with appropriate antiseptic and allows to dry
Step 12
Selects needle (according to size and condition of vein)
Step 13
Reapplies tourniquet
Step 14
Places needle in vacutainer holder and positions tube in holder without breaking vacuum
Step 15
Reassures patient
Step 16
Removes needle cap; positions needle with bevel facing up; quickly inspects needle
Step 17
Anchors vein by placing thumb about 2cm below site of entry and pulling skin downward to keep it tight
Step 18
Angles needle at approximately 15-30 degrees; without contaminations
Step 19
Warns patient about the pinch
Step 20
Inserts needle under the skin and into lumen of the vein
Step 21
Pushes first tube into holder without moving needle (counter pressure)
Step 22
Obtains the proper amount of blood for all tests; allows tubes to fill; removes and replaces tubes without moving needle
Step 23
Uses correct order of draw
Step 24
Mixes tubes accordingly
