define heart failure
heart is unable to keep up with the body’s demands
symptoms associated with CHF, both right and left sided
rapid or slow onset
fatigue or SOB or orthopnea
Left sided: pulmonary edema, wheezing, cough, cyanosis, oliguria
Right sided: edema, jvd, hepatomegaly
What are the underlying causes of Left heart failure
infarct
hypertrophy
What are the causes of right heart failure
usually b/c of left failure R vent infarct pulmonary HTN severe tricuspid regurg RCA and marginal a. infarct
systolic dysfunction
enlarged ventricles fill with blood
the ventricle is unable to pump all of it out so inc ESV
diastolic dysfunction
stiff ventricles fill with less blood than normal
may have normal EF, but CO/EDV just not enough to perfuse tissues
What is the NYHA functional classification?
tells you degree of CHF
What is the major lab for CHF?
BNP
What is Class I CHF?
no limitation of phys act
what is class II CHF
mild symptoms
what is class III CHF
moderate symptoms
limited phys act and comfortable at rest
what is class IV CHF
severe symptoms
symptoms present at rest
unable to do physical activity
