Prescription requirements for veterinary prescriptions [8]

  1. Name. Address. Telephone number. Qualifications and Signature
  2. Name and Address of owner
  3. Identification. Species of animals. Address (if different)
  4. Date - valid 6 months or shorter if indicated. CD are 28 days
  5. Name. Quantity. Dose. Administration
  6. Necessary warning e.g. with food
  7. Under the Cascade
  8. If repeatable, the number of times

A

Animals that belong to species that are normally nourished, bred or kept. They are NOT consumed by humans (except horses)
E.g. Dogs, Cats, Fur animals, Animals in labs, Zoo animals
