Describe the aetiology/ pathophysiology of Fibromyalgia

A

The causes are typically unclear and implicated/ suggested: abnormal neuroendocrine function; decreased BOLD in thalamus, basal ganglia, mid-brain; low insulin; mild hypocortisolemia. DX: > 3 months of widespread pain, pain or tenderness at 11/18 tender-points

Describe the signs and symptoms of Fibromyalgia

A

Deep-aching, radiating, gnawing, shooting or burning, mild to severe pain. Pain spreading through tender points in soft tissue of the: back, neck, shoulders, elbows, sternum, l-back, hips, knees. Joints are not affected. Fatigue and sleep problems are noted in almost all patients

Describe the recommendations and modifications for people w/Fibromyalgia?

A

Low-intensity ex can be well tolerated, however high-intensity depends on individual and needs to be taken with caution; exp should be individualized and focus mainly on long term goals of well-being and maximizing function; low RPE level may be necessary

Describe the responses/ benefits of EXP for people w/Fibro

A

Post-exertional pain may occur, so make sure to monitor patients; ex can reverse the inactivity spiral and further reduce further co-morbidities; adherence is a major problem due to constant pain or thoughts of increased pain; long-term can improve overall function and well-being

Describe the contraindications to EXP for people w/Fibro

A

Contraindications are mainly perceptual and cognitive in that pain can increased pain after exercise and further increase inactivity spiral. Aim to have patients come to their own conclusion that ex is good, and avoid topic surrounding pain
