What is lymphedema?

A

Obstruction of lymphatic drainage resulting in oedema with high protein content

What is the aetiology of lymphedema?

A

1”: Milroy’s syndrome (congenital hereditary lymphoedema)
2”:
-Infection: filariasis, post op
-Malignant infiltration: axillary, groin or intrapelvic
-Radiation/surgery: axillary/groin/lymph node removal

Clinical features of lymphedema?

A
  • Classically non-pitting oedema

- Impaired limb mobility; discomfort/pain; psychological distress.

Mx lymphoedema?

A
  • Avoid injury!
  • Skin hygiene (moisturise, Rx infection)
  • External support (compression bandages/lymphoedema sleeve)
  • Exercise: gentle daily ex whilst wearing compression bandages
  • Massage and manual drainage therapy

Prognosis of lymphoedema?

A
  • Untreated: becomes resistant to Rx due to subcutaneous fibrosis.
  • Cellulitis: rapid increase in swelling. Can lead to sepsis and death.
