A

Date: date order was written

Subject: Nursing orders are instructions to nurses; you don’t have to write “the nurse will….”. It is understood already that this is the order for the nurse and is not necessary to write out those words (“the nurse will”)

Action verb: examples=ausculate, bathe, demonstrate, explain, give, teach, turn

Times and limits: state when, how often, and how long the activity is to be done

Signature: Signing means you accept legal and ethical accountability for your order and allows others to contact you if they have questions or comments about the order
