500-12 Response to Mental Health Incidents

A person with mental illness may not be denied admission or commitment to a mental health facility because the person also suffers from epilepsy, dementia, substance abuse, or intellectual disability.

This form, which is available on the department’s Intranet Portal, does not require notarization. The NED only guarantees an evaluation will be done; it does not guarantee any form of treatment.

a. The person is a person with mental illness and because of that mental illness there is a substantial risk of ____, and;
