Relative afferent pupillary defect
Marcus-Gunn pupil
Electric shock/paresthesia when neck is flexed or extended
Lhermitte’s sign
Hips and knees flex during passive neck flexion
Resistance to straightening the flexed knee
Brudzinski/Kernig’s sign
Indicates meningeal irritation
Percussion at the wrist–>paresthesia in the median nerve distribution
Tinel’s sign
Entrapment neuropathy
Flexion of the thumb (rather than adduction) when pulling a piece of paper between the thumb and index finger
Froment’s sign
Ulnar nerve lesion
Limitation of leg raise to 45-60 degrees
Straight-leg raise test
Nerve root compression
Pressing the heel on the opposite knee causes pain in the hip region
Patrick’s sign
Sacroiliac joint disease
Arm drift with pronation
UMN lesion
Transient hemiparesis after focal motor seizure
Todd’s paralysis
Move the head around and look for nystagmus
Hallpike sign
Inner ear disease–fatigable vertigo and nystagmus after a few seconds
Central lesion–immediate, nonfatigable nystagmus
Retention of pupillary reaction to light in the presence of ptosis and eye movement abnormalities
Ischemic CNIII palsy (diabetes and HTN)
Not seen in compressive CNIII palsy
Tapping thumb with index finger causes other fingers to move synchronously
Fisher’s sign
Lack of other finger movement–>corticospinal tract lesion
Rapidly rolling one arm around the other
Subtle pyramidal tract lesion
Speed of foot tapping
Slow tapping–UMN lesion
Contralateral lid droops when opening or closing the ptotic lid
Herring phenomenon
