Primary HyperAldosteronism - Causes - 4

A
  1. Bilateral idiopathic hyperplasia - 60%
  2. Adenomas - 35%
  3. Conn’s syndrome - more in woman
  4. CYP11B1 and aldosterone synthase fuse, and cells secrete aldosteron in response to ACTH - can be managed with steroids to lower ACTH.

Less then 2 cm
Usually on the left side
Yellow
Cells are filled with fat
If treated with spironolactone can see spironolactone bodies - pink thingy.

Causes 5-10% of secondary high BP, 20% are resistant to treatment.

Adenoma - spironolactone and surgery

Bilateral idiopathic hyperplasia - spironolactone only
