When must a PRC be carried out

Answer 1

All incidents and training events of 6 pumps +

Any incidents at the discretion of of a DAC or above

Any incident involving a Senior Accident Investigation regardless of size

Question 2

Who will attend PRC’s

Answer 2

1/Chair, officer that didn’t attend,up to 8 pumps = GM > 8 pumps or SAI incident = DAC
2/All ICs up to the time of stop
3/Monitoring Officer at time of stop
4/Nominated SAI for SAI events
5/Nominated ORT Officer
6/others deemed to have had role in command e.g. CU team leader
7/ 12 pumps + Control MUST be invited
8/ observers + remote monitoring Officers at discretion of Chair

What resources are to be provided for the PRC

Answer 3

Flip chart/white board/smart board

Plan of incident

Visual aids- tv, video, projector

Photos/ video footage

Copies of the incident report

Question 4

What Roles must chair the PRC

Answer 4

Upto including 8 pumps= GM

Over 8 or SAI incident = DAC or AC

All incidents = to or over 10 pumps must be held at BHQ

Chair must be independent manager who DID NOT attend the incident

Who is responsible for updating the IMPD after a PRC
