Q

BLS Healthcare Provider Adult Cardiac Arrest Algorithm - list the steps until CPR

A

SAH - BnB
1. Safety of scene
2. Assess victim - Are you ok?
3. Help! Call 911, get AED emergency equip
3a. Breathing? Pulse x 10 sec? If so
monitor only until help arrives.
3b. Not breathing? 1 breath every 5-6
seconds or about 10-12/min
* Activate Emergency Response System
* Continue breathing, ck pulse every 2 “
* No pulse? go to CPR

A

SAH BnB CPR AED

  1. CPR 30:2, (use AED as soon as arrives)
  2. AED arrives
  3. Check rhythm.
    • Shockable? Follow AED, give one shock, resume CPR x2” till AED prompts rhythm ck. Continue until pt moves or ALS arrives.

Q

Why avoid interruptions to chest compressions?

A

It takes several compressions to increase blood flow to the heart and brain to levels before the interruption. Interruptions lower the blood supply to the heart and brain.

Q

What are the 2 methods to open the airway?

A
  1. Head Tilt-chin lift, use if jaw thrust does
    not open the airway
  2. Jaw thrust, use if above does not work or suspect head/neck injury as reduces neck and spine movement

16
Q

List the steps to perform head tile-chin lift

A
  1. Place one hand on forehead and push with palm to tilt the head back
  2. Place fingers of other hand under the bony part of the lower jaw near chin
  3. Lift the jaw to bring the chin forward
    (don’t close the mouth completely)
(don’t press deeply into soft tissue under chin, might block the airway)
17
Q

Describe steps to perform a jaw thrust

A
  1. Place one hand on each side of the head, can rest elbows on the surface he is lying on
  2. Place fingers under anglesof lower jaw and lift with both hands, displacing the jaw forward
18
Q

Is a barrier device a standard precaution?

A

Yes, such as pocket masks when giving breaths. Replace face shields with a pocket mask when can

19
Q

Does OSHA require HCW to use standard precautions in the workplace? Including CPR?

A

Yes

20
Q

When placing the pocket mask, what steps do you follow?

A
  • Position yourself at pt’s side
  • Place mask of face, using bridge of nose
    as guide for correct position
  • Open airway with head tilt-chin lift
  • Seal pocket mask against the face
  • With hand that is close to the head, place
index finger and thumb along the edge of
the mask. Place the thumb of your other
hand along the edge of the mask.
21
Q

Continued (pocket mask)

A
  • Place remaining fingers 2nd hand along bony margin of the jaw and lift the jaw, do a head tilt-chin lift to open the airway
  • While you lift the jaw, press firmly and completely around the outside edge of the mask to seak it against te face
  • Deliver each breath over 1 second, enough to make the chest rise, resume chest compressions in <10 seconds
22
Q

Why recommend 2 rescuers to operate Bag-Mask Device?

A

Recommend 2 rescuers, 1 opens the airway and seals the mask agains the face, the other squeezes the bag

23
Q

List the sequence for Adult 2-Rescuer BLS

A
  1. Safety, ck the scene
  2. Assess. Are you ok? Unresponsive?
  3. “Help” Activate EMS
    - Send 2nd rescuer to call 911, get the AED and emergency equipment
  4. Breathing, pulse? (at least 5 sec, no >10
    - “Yes”, monitor
    - “No”, start CPR (EMS should be
    activated by now)
24
Q

If breathing normally and has a pulse, then do what?

A

Monitor only

25
Q

If not breathing but a pulse is present, then do what?

A

Rescue breathing

  • confirm EMS called
  • ck pulse every 2”, CPR if no pulse
  • suspect opioid, consider naloxone
26
Q

If not breathing normally or is only gasping and has no pulse, then do what?

A

One rescuer begin high-quality CPR, starting with chest compressions. 30:2 for victims of any age.

  • use AED when available, follow prompts until support available
  • SWITCH compressors frequently to avoid fatigue, every 2” or 5 cycles, typically when AED is analyzing rhythm. Follow prompts. Resume CPR after shock or no shock advised. Take <5 seconds to switch.
27
Q

2nd step for 1 rescuer

A
  1. Assess. Are you ok? Unresponsive?
28
Q

3rd step for 1 rescuer

A
  1. “Help” Activate EMS
    - call 911 on cell phone
    - Get the AED or send someone
    - Mobilize code team if med setting
    )
29
Q

4th step for 1 rescuer

A
  1. Activate Emergency Response System
30
Q

5th step for 1 rescuer

A
  1. Breathing, pulse? (at least 5, no > 10 sec)
    - “Yes”, monitor
    - “No”, start CPR (EMS should be
    activated by now
