what do you assess in “A” of ABCs
Airway and C-Spine protection
How do we evaluate Airway?
Ask there name!
- Pt communication
- Inspecting for foreign bodies
- Examine for stridor, hoarseness, gurgling, blood ect.
Assume C-Spine with poly trauma…
Clear clinically and radiographicly
B-Breathing…Does a patent airway mean adequate ventilation?
NO
How do we assess Breathing?
Inspect, palpate and auscultate!
What do we look for in breathing assessment (Signs/symptoms?
- Trachea deviation
- Crepitius
- Flair Chest
- sucking chest wound
- Absent breath sounds
What interventions can we do for breathing deficiency
- 100% o2
- Needle decompression
- chest tubes for hemopneumo thorax
- occlusive dressing for sucking chest wound
C- Circulation…who do we assume is in hemoragic shock?
Any hypotensive trauma pts we assume hemoragic shock.
Assessing Circulation
- level of conciseness/ mental status
- Skin color/cap refill
- pulses in all four limbs
- BP & pulse pressure after
Circulation intervention
- Cardiac Monitor
- Stop bleeding (Tourniquets)
- IV Access (2 large bore) or Central line/ IO
- Cardiac Tampanote –> decompress
- VOLUME!!!!!
Volume resuscitation
Blood, FFP, Saline
Criteria for an ED Thoracotomy
- Witnessed loss of pulses
- Ideal pts is low velocity penetrating trauma
D- Disability
Abbreviated neurological exam
