Describe the theory of spreading cortical depression (SCD).

A

A genetically susceptible patient has a multifactorial defect in brain metabolism leading to a gain in NMDA-receptor function (excitatory receptor). Activation leads to a burst of focal cerebral activity causing local hyperemia and positive symptoms (usually in the occipital lobe). Trigeminal nerves wrap around pain sensitive structures, release local neuropeptides, and convey information to the trigeminal nucleus in the brainstem. This creates a feedback loop with the trigeminal pathway.
