Anything that is not included in the prescription that is a legal requirement i.e
Age/DOB if under 12, patient name & address, date (within 6 months), type of prescriber, prescriber address.

Those such as an overdose, undergoes, not enough quantity of medication, incorrect strength. Also issues related to the patient i.e are they allergic to it? Is it appropriate for them? Is the drug prescribed for the correct thing? Duration of treatment? Interactions?

What types of contractual problems can be encountered?

Dental & nurse formulary - they could prescribe something that is not in these formularies and thus they are not able to prescribe it

If an Rx is out of date - what must you do?

Answer 5

Contact prescriber to see if the patient still requires the medication. If so we must request an amended prescription that is in date.
