Expected Alarm
Definition and Required Actions
A Reactor Operator will address another member of the Control Room Staff, THEN announce the alarm title.
IF the alarm is expected due to current operations, surveillance testing, alarm testing, material condition or Maintenance Activities, THEN “EXPECTED” will be announced after the alarm title and the reason for the alarm.
PRIOR TO conducting an evolution or direct operator action, expected alarms should be identified and discussed. IF the action results in alarms that were not identified, THEN they should be responded to as unexpected alarms. (Expectations during tasks and evolutions)
The Reactor Operator(s) will scan the control panels for additional alarms when acknowledging an alarm.
For all expected alarms, a member of the control room staff SHALL review or have previously reviewed during the shift the associated annunciator response procedure to verify alarm input validity and perform required alarm response actions. IF the Control Staff is given an “Unreliable Sheet” during maintenance/surveillance testing that states what the unreliable annunciators are, THEN the Unit Supervisor/Shift Manager will be briefed to ensure the appropriate alarm response actions are being taken.

Unexpected Alarm
All alarms will be considered as unexpected alarms the first time they are received in the shift unless they are the result of direct operator action or meet the requirements of 2.1.1.2.
A Reactor Operator will address the Unit Supervisor/Shift Manager, and then announce the alarm title.
A Reactor Operator will implement the annunciator response procedure.
The Reactor Operator(s) will scan the control panels for additional alarms when acknowledging an alarm.
US/SM will ensure proper actions are taken as directed by the appropriate Annunciator Response procedure.
The Unit Supervisor/Shift Manager will determine whether the alarm can be considered an expected alarm for the remainder of the shift.

When should control bands and guardrails be established ?
When mitigating a transient, it is necessary for the Unit Supervisor to:
Identify the critical parameters necessary for control.
Establish a band within which the critical parameter should be controlled, with a specific owner for each critical parameter.
The Reactor Operator will promptly report as the parameter becomes out of the specified band or is expected to trend out of band (including reason, if known) so that remedial action can be taken and/or a new band established.
Unit Supervisors will assign owners to any key controls that are operated in manual.
Guardrails should be established for manually tripping the reactor and entering 0POP05-E0-E000, Reactor Trip and Safety Injection.

Instrument Response Standards
Personnel must assume instrument readings are accurate unless proven otherwise. Diverse or independent indications, if available, should be checked when unexpected readings are observed.
Operations personnel should record analog meter indications to the nearest half (1/2) of the smallest demarcation (good engineering practice). Questionable Analog Meter readings should be backed up with digital data.

When can or should a controller be operated in manual ?
When specified by Procedures, Maintenance Activities, or because of known material condition issues affecting the automatic controls ability to perform its intended function.
Operators have an obligation to take manual control of any controller that is malfunctioning and causing plant perturbations or to optimize the automatic performance of a controller.
The Unit Supervisor/Shift Manager will evaluate controller performance prior to directing manual operations.
