Vascular headaches:
abnormal reaction of cerebral arteries, particularly a tendency towards dilation
Muscle contraction (tension) headaches:
persistent contraction of the muscles of the head, neck and face.
Traction and inflammatory headaches:
headaches secondary to organic disease of the skull and its components
Idiopathic cranial neuralgias:
no pathologic changes are found
Intracranial pain-sensitive structures:
- venous sinuses
- cortical veins
- cerebral arteries
- dura of anterior, middle and posterior fossa
Extracranial pain-sensitive structures:
- scalp vessels and muscle
- orbital contents
- mucous membrane of nasal and paranasal spaces
- external and middle ear
- teeth and gums
Primary headache:
- tension type headache
- migrane
- cluster headache
- other, rare types of primary headaches
Secondary headache:
- the pathogenesis or the ethiopathology is known
How many of the population are effected by migraine headache?
12%-15%
F:M migraine headache
F:M=3:1 - predominance in women (may subside after menopause)
Migraine and family history:
70% of migraine pts have a family history of the disease
when does the first attack of migraine occur?
First attacks may occur at any time between the ages 5-30yrs (usually begins during puberty)
Migraine personality:
Som migraine patients exhibit a “migraine personality”: they are perfectionist, compulsive, meticulous, ambitious and usually very intelligent.
They also tend to be tense, sensitive, vulnerable to frustration and prone to obsession.
Classic migraine with aura:
- 20%
- focal neurological symptoms precede or, less often, accompany the headache
Classic migraine without aura:
- 80%
- the focal neurological symptoms are absent
