  • Pregnancy
  • Endocrine: thyroid, diabetes, acromegaly
  • Rheum: RA, OA
  • Otrtho: structural abnormalities
  • Workplace factors
  • Drugs: aromatase inhibitors

Clinical manifestations of carpal tunnel syndrome

A
  • Pain/parasthesias in distribution of median nerve
  • Worse @ night
  • Later: weakness of thumb abduction + opposition, atrophy of thenar eminence
  • Can be provoked by Phalen (reverse prayer) and Tinel tests (tapping)

- To rule out others: U/S, nerve conduction studies, rheum screen, diabetes bloods

  • Conservative: rest, night splints, hand therapy
  • Pharmacological: NSAIDS, steroid injection
  • Surgical: carpal tunnel release- open or endoscopic
