  • a medical record must be kept for each patient encounter, regardless of the service provided (inpatient, emergency department, outpatient Diagnostics, Etc.).
  • the medical record must be available at all times.
  • the medical record department is staffed adequately.
  • written policies and procedures are in placed regarding the content in completion of medical records.
  • only authorized individuals (as spelled out by facility policy) shall document in any medical record.
  • medical records shall be completed within a certain period of time following the date of discharge or date of the outpatient encounter.
  • medical records shall be safely stored (though most do not specify in what medium the records must be maintained).
  • medical records shall be preserved (regardless of the medium) for a certain period of time, known as the retention requirement.
