  • Suicide
  • Institutional deaths (Police, Corrections, Oranga Tamariki, Mental Health etc)
  • Cause - where the Coroner is not satisfied of the cause of death

  • Unnatural or violent
  • Medical misadventure or mistake
  • Birth or during childbirth
  • Institutional deaths such as in prison, police custody etc
  • Enactment that requires it

A
  • Verification of Death (VOD) from a nurse, midwife, doctor, advanced paramedic or paramedic
  • A medical cause of death certificate issued by a Doctor (if a Doctor is willing to issue one)
