Barlow’s Syndrome

What can cause Barlow’s?
A
- Marfan’s
- Grave’s
- Rheumatic Heart Disease
- Ischaemic heart disease, HOCM, Dilated cardiomyopathy

How might Barlow’s present?
A
- Palpatations
- Fatigue
- Dyspnoea
- Nausea and Migraines

What murmur may be heard?
A
- Late systolic murmur, progressing to holosystolic murmur as mitral regurgitation progresses

What imaging is used for the diagnosis?
A
ECHO

Q
How are asymptomatic patients treated?
A
- Reassurance of excellent prognosis
- Encourage exercise and healthy lifestyle
- Possibly aspirin or warfarin for stroke risk

How are symptomatic patients treated?
A
Lifestyle- avoid stimulants, alcohol and exercise regularly
Medicine- beta-blocker for symptom control e.g. palpations
Surgery- mitral repair is preferred over replacement due to its lower mortality

How does Barlow’s differ from MR?
A
MVP/Barlow’s is not MR. MVP is the leading cause of MR in the US, but itself describes the floppy nature of the valves
