Pharm. and Action

A

Increases the force of myocardial contraction by initiation of of myofibril shortening; The positive inotropic effect and vasoconstricting effects produce a rise in systemic arterial pressure.

Indications

A

In cardiac arrest setting= Hyperkalemia due to renal failure; Hypocalcemia due to multiple blood transfusions; Known or suspected calcium channel blocker overdose

Contraindications

A

Cannot be administered with sodium bicarbonate; In presence of sodium bicarbonates, calcium salts will precipitate as carbonates

Administration

A

Dose is 2-4 mg/kg IV. May be repeated if necessary.

Side Effects and Special Notes

A

Extremely important to flush the IV line between administration of sodium bicarbonate and calcium chloride to avoid precipitation; May produce coronary or cerebral spasms; Should be used with caution in patients receiving digitalis; may precipitate toxicity
