1. A Summons must:
(A) name the Court and the parties;
(B) be directed to the D
(C) state the name and address of P’s attorney (or P’s if unrepresented)
(D) state the time D must appear and defend
(E) notify D that failure to appear will result in Default Jdgmt. for the amount sought in the Complaint
(F) be signed by the Clerk
(G) bear the Court’s seal

Rule 4(2)(b): Issuance

(b) On or after filing the Complaint, P must present a properly completed Summons for the Clerk to sign, which upon signing, the P must serve on the D (or each individual D if multiple)
