BR- OB Basics Flashcards
Components of BPS
1 nonstress test
- Fetal breathing
- Fetal movement
- Fetal tone
- Amniotic fluid volume
Define acceleration in 32 weeks and above
> 15bpm from baseline, lasts 15 secs, but <2mins from onset to return
Define acceleration for less than 32 weeks
> = 10 bpm from baseline, lasts 10 secs, but < 2 mins from onset to return
Define prolonged acceleration
Acceleration lasts > 2mins but < 10 mins duration
Define a baseline change
Acceleartaion lasts 10 mins or more
Significance of Early Deceleratino
Fetal head compression
SEQUENCE:
- Uterine contraction
- Fetal head compression
- Inc ICP
- Stimultaino of vagal nerve
- Dec HR
Sig of late decel1
Uteroplacental insufficiency
Uterine contraction —> dec U-P O2 transfer -> chemoreceptorstimulus —> stimulation of vagal nerve —> dec FHR
Sig of variable decel
Umbilical cord occlusion
Resuscitative interventions for Cat II/III tracings
- Initial lateral decubitus positioning
- Administer maternal oxygen
- Administer IV fluid bolus
- Discontinue oxytocin or prostaglandins
- Tocolytics, eg terbutaline, MgSO4
