4 PRINCIPLES OF MEDICINE OPTIMISATION

There’s no law against a Welsh language prescription, doesn’t have to have English on it too.
RPS Welsh Pharmacy Board recommends using english to label medicines to avoid errors
Pharmacist who isnt welsh could use a translation if want to but must put safety first for patient if not confident
Prescriptions from Crown Dependencies (Jersey, Isle of Man/ Guernsey) - requirements
All normal prescriptions requirements.
If a Schedule 2 or 3 CD: prescribers address must be in the UK
Repeat prescriptions:
1- if a number of repeats not stated?
2-which schedules of CDs are repeatable
3 - time limits for repeat prescriptions
1- if no number, can repeat once (total dispensed twice). but if its oral contraception, can repeat 5 times (total dispensed 6 times)
2- schedule 4 and 5 are repeatable
3- first dispensing must be within 6 months as normal (28 days for schedule 4 CD), but no time limits for any repeats
Record keeping - private prescriptions: what must records include.
- supply date
- prescription date
- medicine details (name, quantity, formulation, strength)
- prescriber details (name + address)
- patient details (name + address)
Record must be made on date of sale or day after.
Oral contraceptives are exempt.
CD Sched 2 is exempt (becasue will be in CD register)
POM records kept for 2 years
private CD Rx records sent to relevent NHS agency.
How long are prescriptions valid for:
1) POMs and schedule 5
2) P and GSL
3) schedule 2, 3, 4 CDs
1) 6 months
2) 6 months
3) 28 days
Rx requirements for EEA/ Switzerland
- Patient full name and DOB
- Prescriber full name, work address (inc country), phone/fax number, email address, professional qualifications,
- Full medicine details (name, strength, form, quantitiy and dose details)
- Prescriber signature
- date (valid 6 months from when signed)
Even if in foreign language its valid. pharmacist use judgement
Schedule 1,2,3 not allowed
Emergency supply for EEA/ Switzerland
Allowed, prescription needs to be received within 72 hours. Schedule 4 and 5 are ok for this.
