Goal
establish clear policies and procedures for clinical workflow and determine desired handoff procedure for each workflow
Human factor engineering
Working conditions of individuals that strive to design countermeasure practice to minimize errors
Factors that lead to error
complacent, distractions, fatigue, lack of assertiveness, lack of awareness, lack of knowledge, teamwork pressure, and communication
expected bias
expect to find something (ex. missing bolus) therefore being biased towards thinking you found it. (found bolus, assume it is correct but it’s not the proper piece)
Confirmation bias
tend to process information by “looking for” or “interpretation” information that is consistent with one’s existing beliefs. (Use to performing a routine task and miss a change in the plan)
What are the three times of incidents?
Incident, Near-miss, and Unsafe condition
Incident
even that reaches the patient with or without harm
Near-miss
come close to reaching the patient but is caught and corrected prior (circumstance incident that did not occur and was identified prior to treatment delivery)
Unsafe condition
any condition that increases the probability of an incident reaching the patient
Sentinel event
event that causes physical, psychological, and or any great harm from pain. Any unanticipated result death or harm.
Does IFD include any devices?
No, just the patient
What would happen if there is an error in the IFD? (3 answers)
- Calculate to the wrong depth
- Calculate to the wrong dose
- Calculate to the wrong distance
Who can write a prescription?
Physicist, Physician, and Dosimetrist can all write a prescription, however, they vary based on the type of prescription
