What is endometriosis?

A

ectopic growth of endometrial tissue growth outside of the uterine cavity

How does endometriosis present?

A
  • dysmenorrhoea - pain starts a few days before bleeding and stops after bleeding
  • deep dyspareunia
  • sub-fertility
  • LUTS +/- haematuria
  • dyschaezia (painful passing of stool)

What is the gold-standard investigation for endometriosis?

A
  • laparoscopy

- can also treat any plaques there at the same time

How do you treat endometriosis? What if they want to preserve fertility?

A
  • NSAIDS +/- paracetomol for symptomatic relief
  • triphasic COCP (take for 3/12)
  • Mirena coil

preserved fertility

  • GnRH analogues (induced pseudomenopause low oestrogen levels - risk of osteo and meno Sx)
  • lap incision or laser Tx
