Examples of easily missed or referred pain
MI, DKA, lower lobe pneumonia, sickle cell, opioid withdrawal
Classical presentation of renal colic
Comes and goes, CVA tenderness, +/- loins, radiates to groin, testicle, labia, restlessness
Classical presentation of bladder pain
Diffuse, severe pain in the suprapubic region
Prostatic pain
Dull ache felt in the lower abdomen, rectum, perineum or anterior thigh
Urethral pain
Tickling discomfort to sharp severe pain, triggered by micrurition
Small bowel obstruction
Colicky central pain, associated with vomiting 🤢, abdo distension, +/- constipation
Bowel ischemia
Dull,severe, constant , RUQ/central abdo pain, exacerbated by eating
Biliary pain
Intermittent, ruq/epigastric pain, (can be constant) worse after eating fats
Pancreatic pain
Epigastric, radiates to the back, relieved partly by sitting up & leaning forwards
Peptic ulcer
Dull/burning epigastric pain, typically at night, wakes patient, eating makes it worse
Appendicitis
Migratory pain, periumblical moves to right iliac fossa, painful to move
Diverticulitis
Left lower quadrant pain, sometimes rlq
Cholycystitis
RUQ pain, radiates to right shoulder/back, fatty meals
Ovarian torsion
Sharp lower quadrant pain on affected side radiating to leg or back
Aortic dissection
Sudden tearing pain, radiates to back between shoulder blades
