Q

What are some characteristics of mycetoma-granules?

A

A.drainage
B.swelling & redness
C.sulfur granules

1
Q

What are the causes of mycetoma ?

A

Bacteria, but mainly due to SOIL molds

2
Q

What is acid-fast?

A

Physical property of bacteria, specifically their resistance to decolonization by acids during staining procedures

3
Q

Which bacteria is PARTIALLY acid fast, aerobic, and have gram positive filaments breaking up into rods?

A

Nocadia

4
Q

Where are Nocadia bacteria found?

A

In the soil

5
Q

How is Nocadia acquired?

A

Airborne-inhalation of the bacteria or TRAUMATIC IMPLANTATION

6
Q

What may predispose a pulmonary infection, Nocadia?

A

Immunosuppression & cancer

7
Q

What tests are used to dx Nocadia?

A

Gram-stained smear & Ziehl-Neelsen stain, aerobic culture on blood agar plate

8
Q

What meds are used to treat Nocadia?

A

Sulfonamides in high doses or Trimethoprim/sulfamethoxazole

9
Q

What is a type of cavitary bronchopulmonary disease?

A

TB

10
Q

What are 2 types of Nocadia diseases?

A

Cavitary bronchopulmonary disease & cutaneous/subcutaneous infections

11
Q

Which disease caused by Nocadia is a result of traumatic implantation?

A

Cutaneous/subcutaneous infections

12
Q

Which Nocadia disease causes a brain abscess?

A

Cavitary bronchopulmonary disease

13
Q

Which bacteria is anaerobic, gram positive rods to branching filaments, non acid fast?

A

Actinomyces

14
Q

What is actinomyces reservoir?

A

Anaerobic crevices around the teeth and female genital tract

15
Q

How is actinomyces transmitted?

A

Endogenous normal flora

16
Q

How should you treat actinomyces?

A

Ampicillin or PCN G & surgical drainage of pus

17
Q

If mycetoma is in the mouth, what can it cause?

A

Lumpy jaw

18
Q

What is the pathogenesis if actinomysis?

A

Anaerobic growth

19
Q

What is the lab dx for actinomyces?

A

Anaerobic culture in blood agar plate.
“SULFUR GRANULES” often visible in pus
Gram stain & acid-fast stain
(Gram positive and not acid fast)

20
Q

Disease of actinomyces

A

CNS solitary brain abscess most common
Abdominal: surgery or bowel trauma
Thoracic: aspiration w/ contiguous spread
Pelvic: from thoracic or sometimes IUD’s
Cervicofacial (lumpy jaw): dental trauma it poor hygiene
