How does ACTINOMYCOSIS start ?

A

starts as a swelling, which may occur up to several weeks after the trauma, usually within the submandibular region.
The swelling appears first as a firm and indurated lesion and the overlying skin is usually inflamed, firm but may have a bluish color.

How does multiple ACTINOMYCOSIS accesses appear ?

A

Within the swelling multiple abscesses may form with sinuses draining fluid containing yellow granules (sulfur granules)
which appear microscopically as a mass of Gram-positive mycelia and polymorphs

The fibrous walls are typically described as woody

In which diseases ACTINOMYCOSIS appear ?

A

HIV infection
transplantation, chemotherapy
herpes, and cytomegaloviral ulcerative mucosal lesions.

It is also reported in osteoradionecrosis and in patients with systemic illness.

A

Such destructive lesions within the bone may occur or localize at the apex of one or more teeth and

simulate a pulp related infection such as a periapical granuloma or cyst.

What’s ACTINOMYCOSIS treatment ?

A

Long standing fibrosis cases are treated by draining the abscess, excising the sinus tract with high doses of antibiotics.

Long term high dose penicillin, tetracycline and erythromycin have been used most frequently, but the course of the disease is still often prolonged, In addition to this surgical drainage of the abscesses and excision of sinus tract is necessary to accelerate healing.

Immunomodulation, immune stimulation, vaccination with actinolisate and using levamisole cream for skin lessions
