What is amylase ? Function of amylase ?
Amylase is a group of proteins called digestive enzymes which is present in saliva and Pancreatic juice.
It helps with carbohydrate digestion breaking down starch to oligosaccharides and disaccharides.
They are glycoside hydrolases which act on alpha 1,4- glycoside bond.
What is CRP ? from where is it produced ?
C reactive protein is a substance produced by liver early in response to inflammation. Hence called acute phase reactant.
It is not disease specific.
Helps to monitor disease progression and flares.
What biochemical tests to do in acute pancreatitis ?
Serum electrolytes
Serum calcium
Serum albumin
ABG
What blood tests do you do in acute pancreatitis
Full blood count
Serum amylase
Serum lipase (more sensitive, preferred over amylase)
CRP
LDH
A patient with pancreatitis is having a compression over the stomach. What is it ?
Pseudocyst
What non-bacterial organism can cause infection in this patient?
• Viruses (e.g. Mumps, Coxsackie B and hepatitis)
• Parasites (e.g. ascaris lumbricoides, fasciola hepatica, and hydatid disease)
• Bacteria (mycoplasma pneumonia and leptospirosis)
Patient’s hb has dropped suddenly. Cause ?
Rupture of the Splenic artery (GDA, PDA) pseudoaneurysm and hemorrhage
What nonsurgical option to stop the bleeding ?
Interventional Radiology and embolise the vessel/coiling
How do it work ?
Using real-time imaging, the physician guides a catheter
through the artery and then releases clotting agents (coils,
particles, gel, or foam) into the blood vessel slowing the
blood flow and causing an internal injury to activate the
coagulation cascade.
