Maintenance:
Methadone & buprenorphine.
Naltrexone also for maintenance to reduce euphoria from opioids (but will make detox worse by blocking opioid receptors).

Pharmacotherapy for alcohol withdrawal:
Fluids
Benzos (hallucinations/fear)
Chlordiazepoxide (tremors)
Thiamine
Pharmacotherapy for alcohol relapse prevention:
Disulfiram: aversion therapy (intense vomiting if drink alc — avoid cough syrup, mouthwash, sanitizer!) Daily oral pill.
Naltrexone: lessens highs/pleasures of alcohol. Monthly IM!
Why give thiamine in addiction?
For alcohol use.
Short-term memory
Prevents Wernike’s encephalopathy & Kotsakoff syndrome (chronic, 80% permanent)
Pharmacotherapy for smoking
Sm: nicotine, bupropion/Wellbutrin
