Signs of shock:
Hemodynamic monitoring (BP, MAP, CVP) in all shocks
Detect shock early by monitoring
for hypotension, decreased urine output, and
altered mental status.
Hypovolemic shock:
The priority in hypovolemic
shock is to stop the source of bleeding or fluid loss
and then administer IV fluids to restore fluid volume.
Septic shock:
The priority in septic shock
is administering an IV fluid bolus of 30 mL/kg and collecting blood cultures before giving antibiotics.
Vasopressors:
If IV fluids fail to correct
hypotension from septic shock, use vasopressors (e.g., norepinephrine) to achieve
a MAP >65 mmHg.
Anaphylactic shock:
The priority in anaphylactic shock is to immediately administer IM epinephrine.
Cardiogenic shock
Inotropes (dobutamine)
Diuretics
Neurogenic shock
Vasopressors (phenylephrine)
Atropine for severe bradycardia
Obstructive shock Treat underlying cause:
PE: thrombolytics
Tension pneumothorax: Needle decompression
Cardiac tamponade: Pericardiocentesis
