ISMS stands for:
Integrated safety management system.
ISMS in compliance with:
National and international legislations and regulations (EASA, dutch law) and requirements set by other mgt systems and national/international standards (ISAGO, ISO14001 & ISO50001).
Safety =
Condition in which the risk of injury or damage occurring during operation is limited to an acceptable level.
Safety =
State of being free of danger or threat.
Safety mission and ambition =
Aim is to increase our awareness for risks and let this awareness form the basis of our decisions.
KLM 5 safety principles:
- Work safely
- Stick to the rules
- Report unsafe situations
- Help and challenge each other
- Be fit to work.
Never compromise in safety!
With ISMS our approach should be:
Not be REACTIVE but PREDICTIVE (Voorspellend) —> identify risks before they happen.
Realizing a just culture structure
Find the right balance.
Punitive culture: employees wont report in fear of being punished.
Blame free culture: effectiveness of safety mgt diminishes, because people care less.
ETTO
Efficiency thoroughness trade off. Maintaining the balance between efficiency and thoroughness.
Bow tie method
Assess the most influential items in a structured manner.
Threats (preventive barrier) ~> hazard ~> (recovery barrier) outcome.
4 Safety domains
- Operational safety (ops of ac on ground and in air)
- Occupational safety (work and workplace)
- Environmental safety (surroundings of KLM ops)
- Operational security (safeguarding of civil aviation against acts of unlawful interference).
Safety culture within KLM
Crucial for the effectiveness of SMS. SCO is positioned independent from line organization to foster an objective approach towards risk and compliance determination.
SCO
Safety and compliance organization (responsible for development, admin, supervision, promotion and continuous improvement of SMS).
HIRA
Hazard identification and risk assessment (heart of SMS - used ti BE or REMAIN safe).
4 HIRA Phases
- Identify (inputs from 4 safety domains: FDM, audits, observations, safety reports etc)
- Prioritize (first screening of all incoming data. Identifies urgent or not. Very urgent - ERM = emergency response mgt)
- Analyze
- Asses.
3 processes of HIRA
- Event risk classification (ERC) = identify & prioritize
- Safety issue risk analysis (SIRA) = analyze (issue can be reactive / proactive / predictive)
- Auditing
