Allergic contact dermatitis Flashcards
Most common causes of allergic dermatitis on the lips?
gallates, dyes, flavorings, sunscreens, propolis
Common cause of allergic contact derm on the wrist?
Chromates (in leather)
What areas are spared in clothing dermatitis?
Spares folds (axillary vault) and is accentuated where clothes fit tightly
Number one cause of cosmetics dermatitis?
Fragrances, preservatives are second most common
What is the most common cause of airborne ACD?
Compositae
What type of reactions are most tape reactions?
Irritant contact dermatitis
What causes more allergy, pure metals or metal salts?
Metal salts
What test do you do to see if something has nickel?
Dimethylglyoxime test (turns pink in the presence of pink)
Sources of chromate?
dyes (green felt fabric on pool table), yellow green pigment (tattoos/cosmetics), leather, cement, matches and crude oils.
What is the indicator test for Chromates?
diphenylcarbazide spot test
The molecule used for the cobalt release spot test?
disodium-1-nitroso-2-napthol-3,6-disulfonate
What irritant dermatitis do you get with cobalt?
Irritant reaction w/ purpuric pores (not a true rxn really)
What does cobalt commonly cross react with?
nickel and chromate
What type of reaction can mercury cause?
Oral lichenoid reaction
What things does latex cross react with?
bananas, avocado, chestnut, kiwi, passion fruit
Most common glove allergy?
Thiuram (tetramethylthiuram disulfide)
what does thiuram cross react with?
disulfiram
What is the number one cause of allergic shoe dermatitis?
Mercaptobenzothiazole (MBT)
what is the indicator test for formaldehyde?
Lutidine (acetylacetate)
What type of textile has the least amount of formaldehyde?
polyester
What is the number prservative sensitizer in the USA?
Quanternium-15 (it releases formaldehyde over time)
What products are in Kathon CG (methychloroisothiazolinone/methylisothiazolin one MCI/MI)
found in wet wipes
Can you give a child a vaccine with a positive patch test to thimerosal?
Yes! This + result is almost never relevent
What is the difference between fragrance-free and unscented?
The gragrance free is the only one that doesn’t have any fragrance (unscented may have masking fragrances)
What transdermal patch causes the most allergic contact dermatiis
Clonidine
Which corticosteriod group is least likely to cause allergy?
Class C, screening agent is Betamethasone
What allergens are in the PPPASTE family?
PPD, PABA, Para-aminosaliciylic acid, azo dyes, sulfonamides, thiazides, and ethyl anesthetics
Which is more common, irritant or allergic contact dermatitis?
Irritant is more common, 80% of cases vs 20% being allergic contact
How long does sensitization take for allergic contact dermatitis?
Can vary from a few exposures or after years of exposure
How often do you need an exposure to maintain the allergic reaction in allergic contact dermatitis?
Only need an exposure once q3weeks to keep allergic reaction going.
What is the difference between cross reaction and co-reaction for allergic contact dermatitis?
Cross reaction: sensitization to one compound results in sensitization to compounds w/ similar chemical structure (eg: poison ivy and mago peel; neomycin and gentamycin Co-reaction: Sensitization to two chemicals simultaneously bc they are contacted/used together, but otherwise allergy to one wouldn’t result in allergy to the other (ex: nekcel and cobalt; neomycin and bacitracin
What is the #1 flower associated with fingertip allergic contact derm in florists?
Tulips
What type of pattern might you see for allergic contact derm w/ hair products?
Since the scalp is thick and resistant to allergy it will often presetn on the ears and hairline/face
What nail products may contribute to periocular/eyelid dermatitis?
tosylamide>acrylates, formaldehyde, resin, glutaraldehyde, benzalkonium chloride
1 cause of allergic contact dermatitis on the face/neck?
Fragrances, preservatives are 2nd
Most common cause of perianal dermatitis
Lidocaine (hemorrhoid gel) and preservatives (MCI/MI, in wipes).
Most common causes of oral stomatitis (allergic contact)
dental fillings (mercury/gold/amalgam) which give you a lichenoid reaction, eposxy resins and flavorings like mind and cinnamon.
Most common occupational exposures for allergic contact demratitis?
Rubber > nickel > epoxy and other resins >aromatic amines
What type of dermatitis is caused from most tape reactions?
irritant rather than allergic.
In patch testing which compounds can cause a delayed postive patch test (arises after 7 days)?
gold, neomycin, dodecyl gallate, palladium, p-phenyedediamine, and corticosteroids.
Which metal can cause a persisitant psoitive reaction at the site of patch testing?
Gold
What are some key differences between irritant contact dermatitis vs allergic contact dermatitis on histology?
Allergic has more spongiosis, dermal inflammation and eos and also lacks the necrotic keratinocytes.
Which foods have nickel in them that people with nickel allergy should be aware of?
cocoa, licorice, margarine, peanuts, borwn lentils, walnuts, almonst, hazelnuts, beans.
What is the indicator compound for nickel and what color does it turn?
Dimethyglyoxime, turns pink.
What is the indicator compound for chromates?
diphenylcarbazide spot test
What things have chromates?
dyes (green felt fabric on pool table), yellow/green pigment (tattoos/cosmetics), cement, matches, and crude oils (engine aircraft workers and photographers.
