Anemia Flashcards
(24 cards)
Anemia cause decrease in
- hemoglobin
- hematocrit
- RBC count
Causes of high value of hemoglobin/ RBC
-smoking
-meds (androgens, SGLT2 inhibitors)
- dehydration or hypovolemia
- high altitude
MCV
Average volume of RBC
MCH
average hemoglobin content in a RBC
MCHC
average hemoglobin concentration per RBC
RDW
Measure of variation in RBC size
Reticulocyte
Percentage of total RBC or an absolute count
Microcytic Anemia causes with normal or low reticulocyte
- IDA
- Anemia of chronic disease
Microcytic anemia with high reticulocyte
Thalassemia
Hemolysis
Normocytic anemia causes with low or normal reticulocyte
Bleeding( acute)
IDA(early)
Anemia of chronic disease
Bone marrow suppression (CA, aplastic anemia, infection)
Hypothyroidism
Hypopituatarism
Copper or zinc poisoning
Normocytic anemia with increased reticulocyte
Bleeding
Hemolysis
Bone marrow recovery
Macrocyctic anemia low or normal reticulocyte
Vitamin b12 or b9 deficiency
MDS
Liver disease
Meds (methotrexate, hydroxyurea)
Macrocytic anemia with increased reticulocyte
Hemolysis
Bone marrow recovery
Microcytic MCV will be
Less than 80
Normocytic MCV
80-100
Macrocytic MCV will be
> 100
Causes of IDA
Increased loss or reduced gain
Lab results in IDA
Low Hb
Low serum ferritin
Low serum iron
High transferrin/ total iron binding capacity
Causes of IDA
- cows milk in infants instead of formula or breastfeeding
- menstruation/ pregnancy/ lactation
- history of GI blood loss, Melena, hematemesis, hematuria
- celiac disease, h. pylori
- history of blood donations
- use of NSAID or anticoagulants
- colon CA
Causes of B12 deficiency
Pernicious anemia
Gastrectomy or bariatric surgery
Gastritis
Small bowel disease
Pancreatitis
Diet
Drugs blocking absorption ( PPI,metformin , h2 antagonist )
Causes of folate deficiency
- nutitional deficiency due to alcohol, substance abuse, reduced intake
- malabsorption
- drugs (metho, ethanol, phenytoin)
- pregnancy, lactation
- chronic hemolysis
Anemia of chronic disease causes
- infectious
- inflammatory
- CA
- obesity
- DM
- CHF
- critical illness
- severe trauma
Patho genesis of anemia of chronic disease
- reduced iron availability
- immune mediated dysregulation of iron Hemostasis due to cytokines
- immune mediated effects
