Pitocin side effects
Hypotension, myocardial ischemia, pulmonary edema, death
Pitocin rule of three
1.) Give 3 units over 30 seconds and can be repeated two more times over 30 seconds (wait 3 minutes between each dose)
2.) If not effective after 3 doses of 3 units, move to another uterotonic
3.) 3 units/hr for maintenance
Hemabate (Carboprost)
250mcg/ml IM
Contraindicated in asthmatics
Can cause diarrhea
Methergine
200mcg/ml IM
Contraindicated in hypertensive patients (preeclampsia)
Preferred over hemabate
Cytotec
Last resort
Administered rectally
Magnesium
Raises seizure threshold in preeclampsia patients
Nitroglycerine
Treat uterine inversion
Can give larger doses (250-500mcg) than normal cardiac patients
Volatile agents
Can have tocolytic effects so lower volatile agent and increase nitrous oxide after delivery of baby
Normal fetal HR
120-160
Bradycardia fetal HR
<120bpm
Tachycardia fetal HR
> 160bpm
Early decelerations (Type 1)
at the onset of contraction (Vagal head squeeze)
