chronic otitis - purpose of a culture of the ear

A

Culture of the ears is primarily intended to address possible systemic treatment as it does not tell us how an organism would react if it was “bombed” with the antibiotic. Nevertheless, one can certainly use a culture as an aid in the identification of the offending organism.
Culture does not replace cytology in the treatment of otitis.

Osurnia (Elanco) and Claro (Bayer) are designed for long term effect .
- These treatments are administered into the clean ear on 2 occasions, one week apart (Osurnia) or once (Claro) and treatment effect lasts 30+ days.
- It is reportedly formulated in such a way as to allow for distribution throughout the ear canal and diffusion of active metabolites for weeks.

> may occur due to dog head shaking
> Osurnia has been reported in connection with corneal ulcers

The FDA has issues a warning to avoid contact with eyes during osurnia administration. Corneal ulceration and other eye injuries have been reported with accidental ocular exposure to Osurnia.

  • Some ceruminolytics can cause inflammation and partial to complete loss of cochlear function when allowed to enter the middle ear cavity
  • the bottom line is: most ceruminolytics and detergents are contraindicated if the tympanum is ruptured; and if there is any question, always use safe products until you have adequately evaluated the integrity of the tympanum

are ear flushes for cats recommended? why?

A

Cats are very sensitive to the effects of ear flushes, and many will develop post treatment neurological signs regardless of the ingredients used, and I do not recommend ear flushes in cats at all unless absolutely necessary and the owner is properly advised

aminoglycoside ototoxicity effects

A
  • Delayed ototoxicity from topical gentamicin has been documented.
  • Aminoglycoside antibiotics can cause hearing loss and vestibulotoxicity, but this may be dose related. It has been said to cause destruction of cochlear hair cells.
  • Hearing loss can take 6 months to develop and progress for up to 12 months following administration of the agent.
  • Neomycin, streptomycin and kanamycin are also slowly eliminated from the inner ear.

  • Chlorhexidine may cause degeneration of the hair cells in the vestibular apparatus and loss of hair cells in the organ of Corti leading to hearing loss as well as damage to mucociliary clearance function on the tympanic bulla
