When flying an approach, when must VOR/LOC be selected by? Are there any exceptions to this?

By the IAF. 
the exceptions are when flying an 
RNAV approach, 
DME/GPS arrival
When flying a STAR that leads into an approach, the PF can leave map mode until turning into final, but the PM should be in VOR/LOC from the IAF.

How do you load a STAR into the FMS?

A

Ensure airport identifier is last waypoint in FMS.
FPL MENU ARRIVE to load approach, PM to review FPL SUMMARY page
Ensure no GAP or NO LINK entries.

Is there a requirement to review the STAR loaded?

A

Yes, during the arrival briefing PF to read arrival waypoints, bearing and distances. PM to confirm accuracy on their FMS.

Marker Sense switches to low,  
MKR audio on, 
DME to HOLD if required

Flight director kept until after touch down

What the the criteria for a stable approach?

A

Stable by 1000HAT for runway aligned approaches in IMC and 500HAT for all other approaches.
Aircraft on correct path with only small heading and pitch changes
Airspeed Vref to Vref +25
Aircraft in landing configuration
Max 1000FPM sink rate
Normal power setting for config and above idle
Briefings and checklists completed
For circling or visual approach, aligned with runway by 400HAT

When is Vref or Vref+ additives to be achieved by? What additives can be made to Vref?

A

50ft
1/2 gust factor up to 10kts and up to 10kts for Windshear. Additives are cumulative up to 15kts.
Don’t adjust red bug for additives.

A
PF calls "Go around, set power" 
PM sets go around power 
PF calls "gear up"  
PM selects gear up "selected"  then automatically sets altitude pre selector (should be set already) HDG/NAV if RNAV, IAS, Alt sel and calls "HDG/LNAV, IAS,...SET ALT SEL ARMED" 
take off normal from there.

How is a go around with no change conducted? Is there any restrictions?

A

PF calls “going around, set power, no change”
PM sets power and calls “power set” then automatically sets altitude (circuit or circling), HDG, IAS and ALT SEL and calls “HDG, IAS,… SET, ALT SEL ARMED”
Approaching circuit height, PF levels aircraft off and sets approx 40%
