Normal Sinus Rhythm
Regular rhythm, P waves before each QRS
Sinus Bradycardia
Regular rhythm, P before QRS just slower
Sinus Tachycardia
Regular Rhythm, P before each QRS just faster
Sinus Pause/Sinus Arrest
Sudden pause with absence of P-QRS-T, underlying rhythm resumes after pause
Atrial Tachycardia
Rapid rhythm with abnormal P waves, regular QRS
Atrial Flutter
Sawtooth flutter waves
Atrial Fibrillation
Irregular rhythm, no distinct p waves, chaotic baseline
Junctional Rhythm
Absent, inverted, or retrograde P waves, regular narrow QRS
First-Degree AV Block
PR interval more than 20 seconds but consistent, every P followed by QRS
Second-Degree AV Block (Type 1)
Progressive PR lengthening until a QRS is dropped
Second-Degree AV Block - Type II
PR interval constant, then a dropped QRS, unpredictable dropped beats
Third-degree AV block (Complete Heart Block)
No relationship between the P waves and QRS complex
Bundle Branch Block
Hitch in the QRS complex
Ventricular Tachycardia
Wide QRS complexes, regular rhythm, and no visible P waves
Ventricular Fibrillation
Chaotic, no identifiable P, QRS, or T waves
