Wernicke’s aphasia presentation
receptive
“word salad” - sentences that make no sense, speech remain fluent
wernickes asphasia anatomy
superior temporal gyrus
-> supplied by inferior division of left MCA
wernickes area forms speech before sending to brocas area
Broca’s asphasia presentation
expressive
speech is non-fluent, laboured + halting
- repetition is impaired
brocas aphasia anatomy
lesion of the inferior frontalgyrus
- > supplied by superior division of left MCA
comprehension in wernickes vs brocas aphasia
wernickes = impaired comprehension
brocas = normal comprehension
conduction = normal comprehension
wernickes - expressive or receptivee?
wernickes = receptive
brocas = expressive (makes sense)
conduction asphasia
classically due to a stroke affecting arcuate fasiculus
–> the connection between Wernickes + Brocas area
conduction asphasia presentation
speech is fluent but repition is poor
- aware of errors they are making
comprehension = normal
global asphasia
large lesion affecting all 3 areas
- wernickes
- brocas
- arcuate fasiculus
can only communicate with gestures
if speech is fluent (but not making sense) how do you distinguish between diagnoses
is comprehension intact?
intact = conduction (arcuate fasiculus)
impaired = wernickes
if speech is non-fluent (laboured, halting) how do you distinguish between diagnoses
is comprehension intact?
intact = broca’s
impaired = global asphasia!
