(11 cards)
What is the 3 factors that define asthma
- Chronic airway inflammation
- Bronchial hypersensitivity
- Reversible airway obstruction
What are some phenotypes of asthma
- early onset allergic
- Late onset eosinophilic
- Exercise induced
- Obesity
- Neutrophilic
What are some characteristic signs of asthma
- May be worse at night or in the morning
- Usually have triggers
What are the findings on a spirometry for asthma
- FEV1/FVC <70%
- improve in FEV1 or FVC of 12% AND 200ml (or >10%)
What are some findings on a PEF for asthma
- Improvement of >20% or 60L/min after bronchodilator
- PEF with significant daily variation of more than 20% (over 1 week, >10% of over 2 week)
What are some tests for eosinophilic inflammation
- FeNO
- Blood eosinophil
- Skin prick test
What is the first step of treatment in asthma
ICS-formoterol
If asthma is not able to be controlled by ICS formoterol, what is the next step
Add on LAMA (can be tiotropium or 3 in 1) and consider biologics
In primary care, what is the management of mild to moderate asthma exacerbation
SABA 4-10 puff with spacer every 20 min for 1 hour + oral prednisolone 5-7 days
How is mild to moderate asthma managed in ED
SABA with ipratropium bromide and oral corticosteroids
how is severe asthma managed in ED
SABA + ipratropium bromide + oral or IV corticosteroids
May also consider IV magnesium
