what is the ACR criteria for fibromyalgia?
bilateral widespread pain lasting for more than 3 months, 11/18 tender points
what is peripheral pain referring to?
in the region of the nocioceptor, small C fibers actively perpetuate information to region
What is central pain processing?
altered pain perception in CNS, dorsal horn
what is the substance thought to cause tenderness in a neurogenic inflammatory response in peripheral sensitization?
neuropeptides synthesized in DRG (substance P, CGRP etc)
what is wind up?
temporal summation of pain
after central sensitization of pain, what type of afferent fibers transmit pain?
A-B afferents, normally used for proprioceptive input
what is allodynia?
a pain response to a stimuli that shouldn’t hurt
what is hyperalgesia?
a stimuli hurts more than it normally should
what do SNRIs/SSRIs do for the treatment of fibromyalgia? which pain response are they working on?
increase serotonin in CNS in order to prevent efferent pain response (work on inhibitory pathway)
what does pregabalin(lyrica)/anti-convulsants do for the treatment of fibromyalgia?
dampen sensory input from pain stimuli
what is simons theory of trigger point creation?
trauma->sr damage->calcium leakage->inc metabolic activity/decreased circulation->ca can’t get pumped back into SR
what is the function of the vestibular system?
gaze stabilization, postural control, perception of motion
what are the names of the 3 semicircular canals of the vestibular system
Anterior, Posterior, Horizontal
what do the superior nuclei of the vestibular system govern?
VOR
vertical canals
what do the lateral nucleii of the vestibular system govern?
VSR
what do the medial nucleii of the vestibular system govern?
lateral canals

cerebellar
what does the VOR do?
vestibulo-ocular reflex
maintains stable vision during head motion
normal VOR gain=1.0
what does the VSR reflex do?
stabilization of the head and body
what does the COR reflex do?
cervical ocular reflex
impulses from neck afferents cause eye movements
plays a min role in eye control in normal human beings
what does the VCR do?
vestibulocollic reflex
stabilizes the head in space
what does the ccr reflex do?
stabilizes the head on the body
VCR+CCR=protects the c-spine from over rotation
