Q

Primary biliary cirrhosis- autoimmune condition, presents as —- even before jaundice

A

Presents as itching , women, worsens as night due to deposition of bile acids in skin. Chances of mis diagnosis- skin condition

9
Q

PBC primary biliary cirrhosis is with or without extra hepatic obstructions

A

Without extra hepatic obstruction

Ie obstruction is in liver only - jaundice presents late and hence with jaundice poor prognosis

Q

PSC requires annual screening for —

A

Cholangio carcinoma - rare CA of epithelial cells of bile ducts , diff from gall bladder CA

Etiology- 2* to PSC and clonarchis sinensis (parasite) chinese liver fluke infection - eating raw fish

PSC mostly arise as a complication of UC

16
Q

Carcinoid tumour aka

A

Neuroendocrine tumours

Secrete serotonin aka 5HT, 5 hydroxytryptamine , bioactive amine

Other amines - histamine , catecholamines

17
Q

Carcinoid tumour symptoms

A

Diarrhoea - due increased GI motility
Flushing
Increased fibroblasts growth - lesions of rt heart valves - tricuspid and rt pulmonary valvular lesions (carcinoid heart disease)

18
Q

Carcinoid sx

A

Symptoms secondary high serotonin levels- diarrhea and flushing
• Liver and lung metabolize (inactivate) serotonin
• No carcinoid syndrome unless metastatic to liver
• No left sided heart symptoms: inactivated in lungs

19
Q

Carcinoid sx and tryptophan deficiency , vitamin B3 (niacin deficiency)

A

1% body’s tryptophan&raquo_space; serotonin
In carcinoid sx 70% is converted leading to functional deficiencies of tryptophan&raquo_space; vit B3 (Niacin) deficiency&raquo_space; 2* pellagra

20
Q

5 HIAA

A

5-Hydroxyindoleacetic acid
• Metabolite of serotonin
• Appears in urine in carcinoid syndrome
• 24-hour urine sample for diagnosis

21
Q

Most of fats in diet we consume is

A

Triglycerides = glyceride + 3 fatty acids

Pancreatic lipase- breaks down triglycerides in monoglyceride and 2 fatty acids

22
Q

Bile acids secreted by liver from cholesterol

A

Bile salts r Made from bile acids - cholic acid and chenodeoxycholic acid

Bile salts make fat - emulsify (like a soap) and make soluble in water - more area for pancreatic lipase to work

23
Q

Bile acids conjugated by __ to male then bile salts

A

Taurine

Glycine

24
Q

Rate limiting enzyme for bile acid secretion pathway in liver

A

7 alpha hydroxylase

25
Most lipid (triglycerides) absorption occurs through
Jejunum
26
Bile salts are absorbed in
Terminal ileum by active transporters 95% absorbed and recycled- Enterohepatic circulation Only 5% excreted in stool
27
ALP vs AST/ALT
ALP- Alk phos- secreted by Epithelial cells of bile ducts - rise when damage to biliary tree- ie obstruction (cholestasis) AST/ALT- secreted by hepatocytes >> Rise with liver damage
28
Normal values of AST/ ALT and ALP
AST/ALT -50 IU/L ALP- 100
29
Intrahepatic cholestatsis due to -
PBC cholestatis of pregnancy Contraceptive Erythromycin
