CABG surgery
Percutaneous transluminal coronary angioplasty
Stable heart failure caused by cardiomyopathy
Heart transplantation
Valvular heart disease
PAD
At risk for CAD with DM, dyslipidemia, HTN, or obesity
- Other patients that would benefit with prescription from physician and consensus from rehab team
