SOCRATES
Chest pain - systematic review
General: fever, night sweats, rigors, weight loss
Cardio: SOB, nausea, sweating, cough, palpitations, syncope/dizziness, leg swelling
Chest pain - PMHx
Hypertension, MI, hyperlipidaemia, diabetes, lung disease, blood clots
Chest pain - FHx
Cardiovascular/thromboembolic disease
Chest pain - SHx
- Alcohol
- Smoking!!!
- Job
- Living situation
- VTE risk factors if relevant
Palpitations - exploring symptoms
- DOTS + exacerbating/relieving factors
- Rate and rhythm
- Have you ever recorded pulse
- Does it feel regular or irregular
- Does it feel like you miss a beat or have an extra beat
- Can you tap it out
Palpitations - red flags
LOC, dizziness, chest pain, SOB, occur during exertion, sweating, extreme fatigue
Palpitations - system review
- Systemic: fevers, weight change, fatigue
- Psych: low mood
- Neurological: visual changes, motor or sensory disturbances, headache
- Endo: tremor, heat intolerance
- Respiratory: dyspnoea, cough, sputum, wheeze, haemoptysis, pleuritic chest pain
- Gastrointestinal: dyspepsia, nausea, vomiting, dysphagia, abdominal pain, diarrhoea
- Genitourinary: oliguria, polyuria
- Musculoskeletal: chest wall pain, trauma
- Dermatological: rashes, ulcers

- Cardiorespiratory: leg swelling, PND/sleeping upright/pillows, cough/wheeze, chest pain, palpitations, haemoptysis, dizziness, anxiety

- Cardiorespiratory: leg swelling, chest pain, palpitations, haemoptysis
