- Patient details
- Permission: Today I have been asked to perform a cardiovascular examination - this will involve me having a look, a feel and a listen to your chest. We have our examiner here as chaperone. Does that sound okay?
- Exposure - waist upwards
- Reposition - 45 degree
- Any questions? Any pain?
General inspection?
End of bed - ECG leads, Anaemia, scars, visible pulsations, medical paraphernalia, resp rate
Hands inspection
Splinter haemorrhages, Osler’s nodes, Janeway lesions, peripheral cyanosis, tendon xanthomas, perfusion
Hands and arms
Clubbing
Temperature
Capillary refill
CO2 retention flap
Radial Pulse - rate and rhythm, radioradial delay, collapsing
Blood pressure
Face
Eyes - xanthelasma, corneal arcus, anaemia
Mouth - central cyanosis, hydration, high arched palate
Malar flush
Neck
JVP - Hepatojugular Reflex
Carotids - bruits, palpate
Chest inspection
Scars
Axilla
Palpation
Palpation of the Apex Beat
Heaves and Thrills
Auscultation
Auscultation of mitral, tricuspid, pulmonary, & aortic regions
Listen for radiation at carotids and axilla
Listens at lung bases for signs of pulmonary oedema
Accentuation?
- Mitral - left lateral decubitus, deep breath and hold on expiration
Sit forward - Aortic - deep breath, hold on expiration, listen for radiation to carotids
- Pulmonary - deep breath, hold on inspiration
- Tricuspid - deep breath, hold on inspiration
After accentuation (other)
Whilst sat forwards, listen to lung bases for pulmonary oedema and feel for sacral + pedal oedema
