What is entresto? What are its indications?
Sacubitril/Valsartan (neprilysin inhibitor/ARB).
HFrEF (<40%): mortality benefit when replaced ACE-i. Also replaced ARB. Can start entresto instead of either when initiating therapy.
Common indications for ppm placement
symptomatic bradycardia without reversible cause
permanent atrial fibrillation with symptomatic bradycardia
alternating bundle branch block
complete heart block, high-degree atrioventricular (AV) block, or Mobitz type 2 second-degree AV block, irrespective of symptoms.
What is cilastazol? In which patient is it contraindicated?
Phosphodiesterase inhibitor, antiplatelet and vasodilatory properties
Contraindicated with heart failure
Treatment regimen for peripheral artery disease and intermittent claudication
Smoking cessation
Supervised exercise regimen
Statin
Antiplatelet monotherapy (ASA or clopidogrel)
Cilastazol (contraindicated with heart failure)
